Pediatric Pars Fracture (Spondylolysis)

A pars interarticularis fracture—also called spondylolysis—is a stress fracture in a small bridge of bone along the back of the spine. It most often affects the lower lumbar region of adolescent athletes who repeatedly bend or twist their backs in sports such as gymnastics, football, soccer, or dance.
Because the spine is still developing during adolescence, these stress fractures can occur even without a major injury. Early recognition and treatment are key to preventing long-term pain or a more serious condition called spondylolisthesis, where one vertebra slips forward over another.
Symptoms
- Low back pain that worsens with activity or arching the back
- Tight hamstrings or stiffness in the lower spine
- Pain relief with rest
- Occasional pain radiating into the buttocks or thighs
Diagnosis
Diagnosis begins with a thorough physical exam and imaging studies such as MRI or CT scan, which can detect even subtle stress reactions before a full fracture forms. Treatment
Most pars fractures heal without surgery through rest, physical therapy, and activity modification. However, when pain persists or imaging shows a non-healing fracture, minimally invasive pars repair surgery may be recommended to restore stability and motion.
